Description
Provides skilled nursing, physical, speech, occupational therapy, and social work services, as well as home health aides to assist people regain their highest level of health and functioning in the comfort of their own home.

Hours
Monday-Friday, 8:30am-4:30pm except holidays. On-call services are available 24 hr/day with staff available 365 days per year.

Eligibility
Any person in need of the skilled services provided.

Service Area: Cattaraugus County.

Intake Process
Patients should ask their doctor or hospital discharge planner to call Cattaraugus County Health Department Home Health Agency to arrange home care services. Services must be ordered by a physician.

Insurance or sliding fee scale based on income.
