Description
Seeks to improve the system of care for children and youth with special health care needs and their families. The program helps shape public policy so families can get the best health care for their children. A diagnosis is NOT required.
Services include:
- Matching families with community resources and referring to community programs
- Advocacy with health care providers and school districts
- Linkage to medical providers and specialists
- Support and play groups
- Transportation services
- Translation and interpretation services
- Transition to adult services and more

Monday-Friday, 8:30am-4:30pm.
Eligibility
Infants, children and youth up to 21 years old who live in Chautauqua County and have a serious or chronic medical, physical, behavioral, emotional or developmental condition. A diagnosis is NOT required.
