Individual Advocacy, Mentoring Programs, Specialized Information and Referral
Description
Works to empower young adults touched by the child welfare / foster care, juvenile justice, and mental health systems to become active and authentic leaders in their own lives as they transition into adulthood.
Provides the following programs:
- Mentoring & Peer Navigation: Individualized one-on-one mentoring support services aimed at achieving goal attainment through resource navigation within specialized focus areas of independent living, such as housing, employment, education, and social/emotional well-being.
- Placement Support Services: Motivational meetings, individualized mentoring, and treatment planning advocacy efforts aimed at reducing feelings of isolation, bridging service gaps, and uplifting the voices of young adults who are temporarily away from their home community and residing in a foster, kin, group, therapeutic, secured, or congregate care placement setting.
- The Heartline: In partnership with the NYS Office of Children and Family Services, this statewide peer warmline tele-service provides emotional support and resource connections for young adults ages 14-21 who are currently or once were impacted by the child welfare system of New York State.
