Prenatal/Postnatal Home Visitation Programs

Description
Works to provide the education, support, and care needed by families (especially those in Black and Brown communities) to have a healthy start for their babies.

Once enrolled, clients have access to care through home visits from community health workers (CHWs) and registered nurses, who will work with a client's chosen obstetrics provider or help connect them with one. Nurses visit clients throughout their pregnancy to deliver care and answer questions whenever needed. Community health workers support areas of life that affect clients' health, like housing, food, transportation, baby supplies, and preparation for the baby's arrival.

Provides both one-on-one and group educational support so that clients know what to expect at every stage of their parenthood journey. Partners with healthcare providers, doulas, and community organizations to improve outcomes for pregnant persons and their families.

Hours
Monday-Friday, 8am-5pm.

Eligibility
Pregnant persons, mothers with children up to 18 months of age, and fathers and caregivers of infants up to 18 months of age residing in Erie County.
