Description
Provides a caring community that understands the challenges of living with diabetes. Helps individuals to obtain knowledge, skills, and support to better manage prediabetes and diabetes.

Programs include:
- Diabetes Prevention Program: For those with prediabetes or at high risk for type 2 diabetes. A year long program focused on implementing lifestyle changes to reduce the risk of developing type 2 diabetes.
- Diabetes Counseling: For individuals with prediabetes or diabetes. Provides one-on-one visits with a diabetes care and education specialist to discuss individual needs and support.
- Diabetes Self-Management Classes: For any individual with diabetes. Classes focus on the fundamentals of diabetes self-management, covering topics such as understanding diabetes, blood sugar monitoring, nutrition and exercise, acute and chronic complications, and diabetes medications.
- Diabetes Support Group: For any individual with diabetes and those supporting them. Meets once every other month to share struggles, celebrate successes, and discuss topics related to diabetes management.

Hours
Monday-Wednesday, 8am-4pm; Thursday, 8am-6pm; Friday, 8am-12 noon.

Eligibility
Individuals with prediabetes, type 1 diabetes, type 2 diabetes, or gestational diabetes.

Service Area: Genesee, Livingston, Orleans, & Wyoming Counties.

Intake Process
Doctor referral required for most programs.

Program Fees
Fees vary by program; call for more information. Third party insurance, Medicaid, and Medicare accepted.

Support groups are free.
