Description
Provides financial assistance for arrears of rent, mortgage, and electric and gas utilities payments for individuals that are HIV positive and are NOT on a housing subsidy with Evergreen Health. For the Jamestown area, assistance may also cover arrears of sewer service payments.
Hours
Monday-Friday, 8am-5pm.
Eligibility
Individuals who are HIV positive and NOT on a housing subsidy with Evergreen Health who are behind on their rent, mortgage, or utilities payments.

Intake Process
Call for information. Intake will require the following documents attached with application: income verification, housing lease, DVF (diagnostic verification form), and outstanding utility bill / past due rent or mortgage notice.
