Assertive Community Treatment

Description
Provides intensive community treatment to identified recipients with a severe and persistent mental illness. Offers a multi-disciplinary treatment approach which includes clinical treatment of mental illness and substance use, healthcare education and support, psychiatric evaluation and monitoring, family support, service planning and coordination, vocational support, and peer support. Model emphasizes outreach, recovery, relationship building, and individualization of services.

Hours
Office: Monday-Friday, 8:30am-4pm.

Services provided 24 hours a day, 7 days a week.

Eligibility
Persons with severe and persistent mental illness, non-responsive to traditional outpatient services.

Service Area: Niagara County.
