Case/Care Management
Description
Provides Care Coordination for people with intellectual and developmental disabilities in 18 counties throughout the Western New York, Finger Lakes, and Southern Tier regions, with the goal of helping individuals and families navigate a very complex system. Through connections and referrals, Care Coordinators work to make sure that individuals have access to the services and support they need to live a healthy, fulfilling life.
Hours
Monday-Friday, 8am-5pm.
On call 24 hours a day, 7 days a week for emergencies.

Individuals with intellectual and developmental disabilities.
Service Area: Allegany, Cattaraugus, Cayuga, Chautauqua, Chemung, Erie, Genesee, Livingston, Monroe, Niagara, Ontario, Orleans, Schuyler, Seneca, Stueben, Wayne, Wyoming, & Yates Counties.
