Case/Care Management
Description
Provides care management services to Medicaid recipients. Helps Medicaid recipients manage their health care needs and connects them with resources and services, such as food and shelter.
Hours
Monday-Friday, 8am-4pm.
Eligibility
Managed Medicaid recipients of all ages.
Service Area: WNY, as well as the Southern Tier, Finger Lakes, Central New York, & Albany regions.

Free. Care coordination is covered by Medicaid for qualified recipients.
