Provides primary care, podiatry, and school-based health services to the community; see separate listings for dental services.
Hours
Monday-Friday, 8am-5pm.
Eligibility
All community members welcome, specialty service for those with developmental disabilities, I/DD population.
Primary Care & Podiatry: Ages 11 and up.
School Based Health: Locations at Jamestown High School and Washington Middle School
Service Area: Chautauqua County.

Intake Process
Intake packet must be completed first for new patients. Pickup application onsite, call to request by mail, or apply online.
Program Fees
Call with insurance information to ensure coverage. Medicaid and Medicare welcome. Payment plan and sliding fee options also available.
