Description
Provides comprehensive care coordination to alleviate the existing barriers to treatment and support for pregnant and postpartum women with opioid addiction and dependence. Transportation is offered to ensure clients will be able to go the OBGYN, pediatrician, and MAT appointments. Women with opioid use disorders can receive services during their pregnancy and up to six months after giving birth.
M-WRAP services include:
- A Comprehensive Case Management Assessment
- An Integrated Family Case Plan
- A Prenatal Coordinated Care Plan
- Linkage to Appropriate Care
- Home Visits and Regular Contact
- Referral to Supportive Services
- Education and Preparation for Birth
- A Recovery Plan
- Recovery Support Services

Women with opioid disorders, pregnant or up to six months after giving birth, living in Allegany, Cattaraugus, Chautauqua, Erie, Genesee, Niagara, Orleans, Wyoming, & Monroe Counties.
