Medicare
Description
Offers a Medicare Dual Advantage plan for those on both Medicare and Medicaid. A Care Management team helps members with coordinating appointments, labs, tests, transportation, and anything else needed. Members have access to a large provider network, including providers such as Kaleida Health, Catholic Health, ECMC, Niagara Falls Memorial, DENT Neurologic, and many more.
Hours
Monday-Friday, 8am-8pm.
Eligibility
Must be age 21 or older, have Medicare Parts A and B, and have full NYS Medicaid benefits.
