Description
A locally based, non-profit insurance organization that helps people to continue living in their homes rather than moving to a nursing home. Based on an assessment, can provide people with homecare services, home delivered meals, transportation to medical appointments, Day Programs, Therapies, and much more, for no cost.
Hours
Monday-Friday, 8am-5pm.
Eligibility
Must be at least 18 years of age, qualify for Medicaid, and require a "core service" (eg. Homecare, Skilled Nursing, Medical Day Program).
