Transition Services for Students With Disabilities
Description
Provides coaching services to teens and young adults with disabilities and mental health disorders to help them achieve their educational and career goals. Services includes career and post-secondary educational counseling and college enrollment assistance, self-advocacy training, and individual coaching for academic, career, and independent living assistance.
Hours
Monday-Friday, 9am-6pm.
Eligibility
Teens and young adults with disabilities or mental health disorders residing in WNY.
Intake Process
Complete the Request Services form available on the website, call, or email.
Program Fees
Services may be free through Medicaid Self-Direction, ACCES-VR, or for eligible youth in Erie County. Otherwise, fee-for-service is provided on a sliding scale based on income.
