Transition Services for Students With Disabilities
Description
A transition program that provides coaching to teens and young adults with special needs who would like to attend post-secondary institutions, such as college or vocational school. Students may also receive assistance with high school equivalency or transition planning for post-high school. Students may receive academic, life, vocational, and social skills support, including workshops, tutoring, and peer mentoring.
Hours
Monday-Friday, 8am-6pm.
Eligibility
Teens and young adults with disabilities or mental health disorders residing in WNY.
Intake Process
Complete the Interest Survey available on the website or call to be contacted by the Transition Coordinator.
Program Fees
Fees vary depending on services and family income. Accessible Academics is also a vendor through Medicaid Self-Direction as a transition program and ACCES-VR.
