Description
Provides information, assistance, and access to long term supports and services for individuals of all ages with a single phone call. Assists with applications for SNAP, HEAP, and medical insurance (Medicaid & NYSOH Market Place). Independent Living Centers and Senior Services/Office of the Aging work together to help seniors, caregivers, and individuals with disabilities access services and programs through information and referral and person-centered options counseling.
Hours
Monday-Friday, 8:30am-5pm.
Eligibility
Serves individuals of all ages and stages of life including those with acute and chronic health conditions, mental health issues, substance abuse disorders, and all disabilities, as well as their caregivers, family members, and professionals seeking additional information and support for clients.
