Case/Care Management
Description
Children's health home program providing case management services for children ages 0-21 years old. Helps connect to social and medial supports in the community and provides assistance with Department of Social Services and existing community resources.
Hours
Monday-Friday, 8am-5pm.
Eligibility
Children ages 0-21 years old who are Medicaid eligible and have two or more chronic conditions or one single qualifying condition (serious persistent mental health, HIV/AIDS).
