Description
Systemic engagement program that provides crisis intervention and works with a system of care towards crisis prevention and stabilization for individuals with developmental disabilities and co-existing mental health or behavioral health needs. Supports include a 24-hour crisis line, clinical assessment, development of a crisis plan, and integration of tools such as therapeutic coaching.

Hours
24 hours a day, 7 days a week.

Eligibility
Ages 6 and above. Must be enrolled in Medicaid and eligible for OPWDD services.

Serves 17 counties of Western New York including Allegany, Cattaraugus, Chautauqua, Chemung, Erie, Genesee, Livingston, Monroe, Niagara, Ontario, Orleans, Schuyler, Seneca, Steuben, Wayne, Wyoming, & Yates Counties.
